At Supportive Access, we believe that every mind matters. As an experienced registered mental health nurse I am here to support you to access the services or support you need to build a meaningful and fulfilled life.

With over 10 years experience in senior nursing roles in various community and inpatient mental health services, I have a wealth of knowledge to help you.

  • My experience includes: Acute Adult Mental Health Inpatient, Offender Health, Eating Disorders, Older Person's Inpatient, Drug and Alcohol Services, Community Assessment and Triage Services, Specialist Womens Services and DWP.

So if you're looking for an honest and supportive assessment of your needs, to support consultations with your GP, accessing appropriate community support, weekly support discussions, support for adjustments at work, or simply feel you need support but don't know where to turn. I can help! I can also help to support disability claim applications such as PIP. 

I am able to provide assessment, signposting, follow up support and report services to individuals in the Shropshire, Staffordshire, Wrexham and Cheshire area. For those requiring support with DWP applications, this service is offered nationwide. 

All appointments are held remotely. This can be through either Microsoft Teams, or via telephone. 

Services

Full Bio Psycho Social Assessments

After an initial discussion, I can provide a full bio-psycho-social assessment. This gives healthcare professionals a deep understanding of what your needs are and why. This helps to identify what services would be best suited to support you, and to help you access other organisations that may be able to further assist. Up to 6 weekly follow up calls can be arranged, following assessment.

 

Supporting Reports

If you need a report for any reason such as for your GP practice, to support your mental health consultations and access NHS services or to help with aiding your manager in work to make appropriate reasonable adjustments, where you do jot have access to occupational health. A supporting report can be provided following an assessment.

 

DWP Support Package

Whether it's help with filling in your application form, or you need an assessment and supporting report for applications relating to mental health conditions, I can help you with this.  An initial discussion is held prior to this, to ensure you are eligible to apply for these benefits to save you paying for further support for your application if you're unlikely to be awarded.

How do I make an appointment?

Will I recieve a copy of my assessments and/or reports?

Yes, you can request to recieve these via email or post.

Is my information kept confidential?

Yes, all information shared in sessions is kept confidential as part of our professional commitment to your privacy, unless there is any information shared that would require me to initiate safeguarding proceedure. 

Can you share my assessment and reports with my GP?

Yes, if you wish for me to do this I can share this with your registered GP.

What are your prices?

Each individuals circumstances are different. If you'd like information on prices and any concessions available, please contact me via email or the contact form.

What ages do you work with?

For full mental health assessments and supporting reports I can work with anyone aged 18 years plus.

 

For DWP disability support I can work with anyone aged 16 and over, however, I request that parents contact details are provided.

Do you offer emergency mental health support?

I do not offer emergency mental health support. You will need to contact out of hours support services, emergency services or crisis charitable organisations such as the samaritans.
